‘My Eyes Were So Swollen I Was Unable to Even Open Them’

Amanda Preisinger feels anxious about her child’s impending 13th birthday party. It’s not due to the typical reasons related to a house full of loud adolescent kids, but because it’s the first time she will debut her recently altered face to acquaintances and relatives. “Obviously I’m going to tell everyone as they come in, ‘For your information, this is not how I look,’” states the thirty-year-old property agent from Southern Florida.

How she looks is, admittedly, a somewhat startling – her face puffed up and unnaturally tightened, as though secured by heavy-duty tape. Her new – and she’s eager to emphasize, temporary – look is the result of half a dozen cosmetic procedures, including an minimally invasive facelift, performed by a doctor in Istanbul, Turkey, last month. “My poor husband teared up when he viewed me for the first time because I wasn’t able to even unseal my eyes. That indicates puffy I was,” she tells me via video call from her house. “I needed to inform him: ‘Babe, I’m okay, I’m not in pain. I just appear as if someone jumped me.’”

Increasing Popularity of Early Facelifts

Based on cosmetic specialists, Preisinger is among a rising count of people electing to undergo a facelift in their twenties and thirties – well over a decade before typical surgeons’ typical patient age range of forty to sixty. (Although most surgeons are keen to emphasise the industry edict of: “We address heredity, not age.”) “I have individuals in their late twenties asking for facelifts,” states London-based cosmetic surgeon Georgios Orfaniotis, a ex-NHS doctor who specialises in the head and neck. “It’s a major surgery, and I’m a somewhat worried when I observe individuals opting for it as a personal preference.”

Explaining the Facelift Procedure

A rhytidectomy, alternatively called a facelift, elevates the ligaments in the face that begin to droop as we grow older. “Our faces are structured a little like onion layers,” explains Kent-based face specialist Marc Pacifico. “Dermis and adipose on the surface, then a stratum of connective tissue called the SMAS. Think of the SMAS as the structural foundation of the face. And that’s what causes aging; that is what becomes lax and drags the skin downward with it.”

You risk performing surgery on people that have deeper issues. It’s not a practice for an conscientious surgeon to follow

Once the preserve of moneyed older people, overuse of injectables – when excessive use of injectable gels expands the face and causes looseness in the dermis – alongside the common adoption of slimming medications, low-cost medical tourism, and the development of novel, celebrity-endorsed operative methods are driving a new kind of patient towards this major operation. Reports show an eight percent rise in facelifts over the last year in the UK; while a study revealed that the portion of youthful candidates is growing, with one-third of procedures now conducted for patients aged 35-55.

“Patients are asking now to look like the optimal form of themselves and obviously the techniques are evolving,” comments Orfaniotis. Currently, the trending procedure is the comprehensive facial lift, a method promoted by figures including Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Deep Plane Technique

Where a conventional facelift entails lifting the SMAS, the comprehensive lift releases the underlying structures underneath, allowing doctors to restructure the face by moving the underlying layers, and preventing the taut, pulled appearance occasionally caused by older methods. “We avoid placing stress on the dermis because we’re going below, to the deeper tissue,” says prestigious London plastic surgeon Rajiv Grover, who focuses on the deep plane technique. “It is the deeper tissue that is adjusted. The dermis follows along as a passenger.” Lifting the whole facial structure as a single unit allows for a more natural-looking and longer-lasting outcome. It also means that the procedure is no longer being used for the express purpose of anti-ageing – or rejuvenation, as the aesthetics industry terms it – but for “enhancement” and facial restructuring, too. “We receive many inquiries from clients aged 28 to 35 for this reason,” says Grover.

Personal Journey

Preisinger had never planned on undergoing a facial lift – at least not at the age of 30. But years of using hyaluronic acid dermal fillers in an effort at “facial balancing” implied that by the time she reached her late twenties, she was “botched”. “I didn’t do this because I wanted to look young,” she says. “I underwent it because the injectable harmed my face. I regret ever using it. If I had avoided the filler, I don’t believe I would be where I’m at right now.”

Whether injectable substances completely disappear has historically been a point of contention in the aesthetics industry. “Research have shown that they seldom completely dissolve,” states Grover, “but they migrate and can obstruct lymphatic channels. A contributing factor to what we term ‘puffy appearance’ is additionally the reality that in a sense, the face gets somewhat fluid-filled because its ability to drain lymphatic fluid has been diminished.” Though research into the area is early-stage, cautionary statements on dermal fillers’ possible effects on the lymphatic system have been issued, and additional studies initiated. An esteemed face specialist, speaking on the condition of anonymity, mentioned he would never use injectables in a client because of the dangers they pose. “A lot of surgeons don’t want to talk about this, because the manufacturers who make filler can be pretty aggressive.” He’s heard stories, he says, of plastic surgeons being landed with court orders after raising their concerns.

Choice and Surgery

For Preisinger, after beginning using injectables, she believed she needed to continue replenishing it – especially as it began moving to different areas of her face. She was just 28 when a nearby cosmetic doctor in the state suggested that a facial and neck surgery might be what she needed to finally step off the cycle of treatments. After 24 months, she found herself selecting from a list of recommended hotels in Istanbul, texted to her by the patient coordinator at her surgeon’s office.

Originally, Preisinger had planned for an upper and lower blepharoplasty – better known as eyelid surgery – but her doctor recommended that a facial lift was the correct option for the laxity she was experiencing in her face. She arranged a forehead lift, a cheek lift and a jawline surgery. The eve of her operation, 24 hours post she’d arrived solo in Turkey, she decided to add a mouth enhancement. “Then he said, ‘We will lift the under-eye bags.’ Then he said, ‘I believe if we remove some cheek fat, it will sculpt your face.’ Thus I ultimately including additional procedures,” she says. She paid $22,000 (sixteen thousand five hundred pounds) for the six-hour surgery, plus a three-day|
